The idea is that one of the rare faction spawns that can drop extra-valuable loot will be more likely to appear in an asteroid belt if there are no wrecks present. There is nothing to substantiate this idea.
I don't know of any asteroid (not ice) belts that put you 100km out if you warp to the belt at 0km. Typically it'll be between 20km-50km. If that's too far to fly, just bookmark the asteroid you want to mine, warp to a nearby planet, and then warp to the bookmark at 0.

To the best of my understanding, if you completely mine out a belt, it will respawn, but with a lesser amount of ore per asteroid. The more often a belt gets totally depleted, the lower the rate.
When you mine an asteroid down, whether it be to a pebble or nothing at all, its ore gets respawned elsewhere in the EVE world. It will regenerate in your own system when it's depleted elsewhere, though due to the mechanics of the regeneration (which I can't go into) it may take a while for the asteroid to resume its former size.
